Established to give comfort, honor and dignity for end-of-life patients and their families through compassionate physical, emotional and spiritual care, regardless of their financial circumstances. We help our valued patients, and their families experience the fullness and dignity they deserve, even as they face the anxieties associated with the end of life.

Hospice Care of the Lowcountry is seeking a Registered Nurse Case Manager to join us full-time, Monday through Friday, 8:30-5:00. The successful candidate will be responsible for patient care and coordination with the interdisciplinary team. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s physical, emotional, and spiritual needs. Come enjoy manageable caseloads and the ability to provide quality care based on patient need. Previous hospice case management or home health experience is a plus but not required. We cover Beaufort county and Jasper County. Dependable transportation is a must. We look forward to meeting you!

Responsibilities

  • Coordinate and provide care that is safe, timely, effective, efficient, and client-centered
  • Handle case assignments
  • Facilitate multiple care aspects (case coordination, information sharing, etc.)
  • Help patients make informed decisions by acting as their advocate
  • Develop effective working relations and cooperate with the interdisciplinary team throughout the case management process
  • Required documentation - must be comfortable using an iPad
  • Adhere to professional standards as outlined by protocols, rules and regulations

Skills

  • At least 1 year of experience as a RN
  • Effective communication skills
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Compassion
  • Current RN license, BLS certification

**Sign on bonus agreement will be signed for disbursements according to terms

Job Type: Full-time

Pay: $70,000.00 - $83,200.00 per year
