Join our dedicated team at Hospice of the Piedmont as a Palliative Care Nurse Practitioner. We are committed to providing the highest level of skill, compassion, and respect to our community through hospice and palliative care services. As a Palliative Care Nurse Practitioner, you will play a crucial role in providing direct care and clinical expertise to individuals and families facing serious illness and loss. Collaborating with our Medical Directors, NP colleagues, and other staff, you will be responsible for assessing, planning, implementing, and evaluating the care of our hospice and palliative patients.

Responsibilities/General Duties:

  • Provide palliative care visits and coordination of care to patients in various settings, including home, ALF, SNF, and clinic settings.
  • Collaborate with referral sources and PCP to ensure quality services to patients and providers.
  • Assist in outreach activities to promote and educate the community about hospice and palliative medicine services.
  • Facilitate and document clarification of patient and family goals of care.
  • Assist with diagnosis, treatment, and management of acute and chronic health conditions.
  • Participate in community education through rotations with nursing and medical students.
  • Consult with the referring physician, hospice team, and other providers to contribute to the patient's plan of care.
  • Prescribe medications and order diagnostics/treatments in collaboration with Medical Directors and referring physicians.
  • Attend and participate in internal meetings and provide educational support to the interdisciplinary group.
  • Adhere to ethical behavior, professionalism, and confidentiality standards.
  • Participate in on-call responsibilities and rotation for 24/7 medical coverage.

Qualifications:

  • Licensed to practice in the state of Virginia
  • Certified as a Clinical Nurse Specialist or Nurse Practitioner
  • Certification by a national body (ANCC, NBCHPN, AANP)
  • Minimum of five years nursing experience; clinical nursing care in geriatrics, long term care, and/or hospice/palliative
  • Palliative care ARN, preferred
  • Knowledge of the interdisciplinary team concept
