Community Health Worker

Job Description

The School of Medicine, General Pediatrics is seeking a Community Health Worker (CHW).  The CHW will report to the Hopkins Community Connection Program Coordinator at the East Baltimore Medical Center. Activities for this position will include, but not be limited to: screening and enrollment for eligible community programs; patient mentorship; patient education; patient follow-up and reinforcement of treatment plan tasks; coordination of appointments and transportation; and patient location and engagement. The CHW will foster cohesive relationships with staff and clinical teams in the area where deployed, with community partners. This position will work with an interdisciplinary team of physicians, nurses, case managers, social workers, dieticians and other community health workers to improve health outcomes. The CHW will be active in community engagement activities to include working with community organizations and assisting with community-based activities. The CHW must have reliable daily transportation for work related travel (to and from community site, patient homes, meetings, clinics, etc.).


Specific Duties & Responsibilities


Outreach and Care Management

  • Assist clients in meeting selected care plan components as assigned. Provide outreach and care management support to patients. Conduct Home Visits as needed, delivering supplies to address food insecurity as indicated.


Patient Advocate

  • Educate patients of available resources. Assist clients in obtaining support when necessary. Act as a patient advocate and support the development of problem-solving skills; provide instructions in basic health care procedures.


Appointments

  • Provide feedback relevant to improving service accessibility and acceptability to providers. Accompany clients to scheduled appointments and/or referral sites, as needed. Make home visits and other contacts with clients, as necessary.


Administrative and Other Duties

  • Maintain necessary forms outlining services provided, neighborhoods visited, and problems encountered. Perform other related duties incidental to the work described here in. The above statements describe the general nature and level of work being performed by individuals assigned to this job.

Special Knowledge, Skills, & Abilities

  • One year of experience in a position involving patient/public contact.
  • Requires working knowledge of social and health issues.
  • Familiarity with medical terminology.
  • Excellent interpersonal and communication skills to efficiently work with diverse groups of community members and to coordinate with team members.
  • Critical thinking skills to effectively assess social/health needs of members and to arrange for appropriate services required to meet those needs.
  • Excellent organizational skills, flexibility, and the ability to get things done.
  • Requires analytical skills to effectively address and evaluate the health education needs of customers.
  • Ability to work independently and efficiently.
  • If using personal vehicle for work related travel, a valid Maryland driver’s license is required.



Minimum Qualifications
  • High School Diploma or graduation equivalent.
  • Two years related experience.
  • Additional education may substitute for required experience, to the extent permitted by the JHU equivalency formula.


Preferred Qualifications
  • Bilingual in English and Spanish.
  • Familiarity with non-profit sector and clinic settings a plus.
  • Passion for HCC’s mission and vision and working with emerging leaders.
  • Exceptional initiative and attention to detail.
  • Superb interpersonal and communication skills.
  • Strong skills in Microsoft Office Suite, especially Teams, Excel and PowerPoint.
