Family Health Centers of San Diego (FHCSD) is looking for a Coordinator of Prenatal Case Management. The Coordinator of Perinatal Case Management oversees the day-to-day operations of perinatal case management activities. In addition to program oversight, the Coordinator provides direct Case Management services to patients as needed.

Responsibilities:

* Builds positive relationships, inspires others to perform at a higher level, prepares career development plans, and retains an accountable workforce.

* Coaches and counsels staff and administers appropriate disciplinary action promptly.

* Gathers program data, conducts analysis, and provides reporting, including productivity reports.

* Maintains a quality monitoring program, conducts quality assurance checks, and ensures standardization of case manager work, including chart reviews, patient satisfaction surveys, staff training and development, and other measures. Ensures the program complies with program requirements.

* Oversees daily case management activities at assigned sites, including scheduling to ensure maximum utilization and productivity, ordering supplies, and maintaining intake enrollment levels.

* Participates in continuous improvement activities and ensures appropriate Lean tools (i.e., 5S, A3) are deployed and results are evaluated and/or measured.

* Performs other duties as assigned.

* Provides case management coverage as needed.

* Provides/ensures staff receive all new hire and annual trainings on time.

Resquirements:

* 1 year experience in maternal child health, perinatal care, and staff supervision preferred.

* 5 years of progressively responsible positions in the healthcare industry or outpatient medical clinics required.

* Ability and means to travel as needed in a timely manner within San Diego County. DRIVER REQUIREMENTS: Requires an active Class C California driver's license, proof of liability insurance at $100,000, on vehicle used. No more than 2 points in past 12 months; No suspensions in last 2 years for moving violations; No DUI, reckless or felony Driving within 5 years. No license revocation in 7 years.

* Bachelor's degree in Health Science or Social Science field required.

* Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
* Ability to work effectively with patients from diverse social, cultural and economic groups.

*Ability to work well in both a team-based environment and independently.

* Bilingual in Spanish/English required.

* Demonstrated ability to be culturally sensitive and respect diversity.

* Intermediate computer literacy ability to comply with department needs and expectations, including MS Office.

* Intermediate interpersonal and customer service skills.

* Intermediate written and verbal communication skills.

* Knowledge of perinatal programs, care management, and procedures.

* Knowledge of supervisory practices and techniques, including performance management and employee development.

* Strong analytical and problem-solving skills.

* Successful track record of leading and driving positive change through others.

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
