Job Roles
- Identify participant's overt problems/needs and set prioriincorporatesties.
- Develop a plan of care based on the physical therapy process which incorporate the plans of other disciplines and continuing care needs to include the participant/family in developing or revising the plan of care. Make referrals to interdisciplinary support services.
- Establish realistic and measurable goals and set up treatment plans to administer rehabilitation treatment. Communicate participant changes to the interdisciplinary team.
- Collect participant data and complete required forms to ensure compliance with regulatory requirements.
- Understand and demonstrate respect for participant's rights and utilize established mechanisms for the management of ethical issues.
- Make home visits for necessary home-based interventions and observations.
- Integrate cost-effective measures into rehabilitation practice. Active involvement and participation in QI processes in complying with policies addressing safe working conditions.
- As an active and cooperative member of the interdisciplinary team, attend meetings and actively participate in the care planning of the PACE participant.
- Perform other duties as assigned.
Requirements:
- Current California state licensure as a Physical Therapist.
- Bachelor's degree in Physical Therapy; Master's degree in Physical Therapy preferred.
- Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
- Ability and means to travel as needed promptly within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.
Experience/Specialized skills
- 1-2 years of experience working with the older adult population.
- Clinical competency with the older adult population is preferred.
- Knowledge of appropriate and updated therapy equipment and technology to include aerobic and strength conditioning equipment tailored to the older adult population and use of mechanical assist devices.
- Ability to educate other staff on proper body mechanics.
- Ability to work effectively in a team environment.
- Demonstrated ability to communicate effectively and openly with other interdisciplinary team members on patient status and needs.
- Must be able to multi-task, be flexible, ensure accuracy, and meet changing priorities in a fast-paced, high-workload environment.
- Basic computer literacy to comply with department needs and expectations (i.e., E-HR documentation, obtaining background information and reports, following up on appointments, etc.).
- Bilingual English/Spanish preferred.

78,416.00 - 111,837.00If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
